Real-time cine first-pass perfusion imaging enables rapid detection of functionally significant high-grade coronary stenosis

چکیده
Background Among the spectrum of patients with coronary artery disease (CAD), those suffering from high-grade stenosis are at a significantly elevated risk for adverse events [1]. Such high-grade disease may result in the so-called “coronary steal” phenomenon under vasodilator stress [2] thereby inducing wall-motion abnormalities (WMAs) in the affected territories, referred to as transient ischemic dilation. We developed a “real time” cine first-pass perfusion (FPP) CMR method for concurrent imaging of myocardial function and perfusion. We hypothesized that this method is capable of simultaneously capturing stress-induced perfusion defects and WMAs in a single ungated scan.

Safety of adenosine stress perfusion cardiac magnetic resonance imaging in patients with aortic stenosis
Methods Consecutive patients with known or suspected coronary artery disease (CAD) and native aortic valve stenosis (aortic valve area [AVA] <2 cm by planimetry), undergoing adenosine perfusion CMR were eligible for enrollment. CMR imaging included cine, first-pass perfusion at stress and rest, and delayed-enhancement imaging. CMR images were interpreted for CAD using an algorithm previously de...
متن کاملWhole-heart dynamic three-dimensional magnetic resonance perfusion imaging for the detection of coronary artery disease defined by fractional flow reserve: determination of volumetric myocardial ischaemic burden and coronary lesion location.
AIMS Dynamic three-dimensional-cardiac magnetic resonance (3D-CMR) perfusion proved highly diagnostic for the detection of angiographically defined coronary artery disease (CAD) and has been used to assess the efficacy of coronary stenting procedures. The present study aimed to relate significant coronary lesions as assessed by fractional flow reserve (FFR) to the volume of myocardial hypoenhan...
متن کاملUngated cine first-pass CMR for concurrent imaging of myocardial perfusion defects and wall motion abnormalities
Background Combined assessment of wall motion from cine imaging and perfusion defects from first-pass perfusion (FPP) imaging has been shown to have a high diagnostic performance for detection of acute ischemia. In this setting, a single ungated CMR scan capable of simultaneously capturing perfusion deficits and wall motion abnormality can be useful for rapid diagnosis of ongoing acute ischemia...
متن کاملEffects of hyperaemia on left ventricular longitudinal strain in patients with suspected coronary artery disease
AIMS Myocardial perfusion imaging during hyperaemic stress is commonly used to detect coronary artery disease. The aim of this study was to investigate the relationship between left ventricular global longitudinal strain (GLS), strain rate (GLSR), myocardial early (E') and late diastolic velocities (A') with adenosine stress first-pass perfusion cardiovascular magnetic resonance (CMR) imaging. ...
متن کاملRapid detection of coronary artery stenoses with real-time perfusion echocardiography during regadenoson stress.
BACKGROUND Real-time myocardial contrast echocardiography permits the detection of myocardial perfusion abnormalities during stress echocardiography, which may improve the accuracy of the test in detecting coronary artery stenoses. We hypothesized that this technique could be used after a bolus injection of the selective A2A receptor agonist regadenoson to rapidly and safely detect coronary art...
